6)*If opting for registration in case of being lost; include the address OR telephone number, or vet name and telephone number. We do not provide rabies vaccination numbers.

*Please keep in mind though, there is a limited space for this.

If you have any other special requests not mentioned, please don't hesitate asking by email. We will do our best to accommodate your request. All personalized orders must be paid in advance, so please Check-Out before emailing us. You can enter all your selections on the Check-Out page so you do not need to email us if you don't want to. We'll get all the info we'll need for doggie!
